In case you want to get your own FFL, you are required to fill out an ATF (Alcohol, Tobacco, Firearms and Explosive) form personally in presence of an FFL License holder. This is a form on yellow sheet that contains name, address, date of birth, government issued photo-id, make/model/serial number of the firearm and an affidavit, obtained from federal law, stating eligibility of the purchaser, as per federal law. Once you have filled out the ATF form, an FBI NICS background check follow.
The ATF form can be downloaded from the website of B. A. T. F. (Bureau of Alcohol, Tobacco, Firearms and Explosive). You may also request them to mail you an application form. Once you have got your ATF, you can order FFL Kit and fill it out carefully. And submit it with due fees. Fees amount is different for different types of licenses.
